The board says previous approaches focused on voluntary compliance have proven ineffective at addressing the unique hazards of flying in the area.

The NTSB says a key component appears to have failed before the Otter crash in Washington, and the agency is calling on regulators to require operators to inspect their planes before their next flights.

The information may shed new light on a crash near Skwentna that killed Anchorage pilot Janell Rude and a Fairbanks crash killed wilderness guide Jerald Stansel.

The wiring was installed as part of a federally funded experimental safety program in Alaska that began in the late 1990s.

The airline sent a second plane from Bethel to pick up the pilot, the passengers and their luggage from the lake.

According to the report, the pilot noticed that the plane’s emergency locator transmitter had turned on shortly after take-off, and he started to smell what he described as “an electrical burn smell.”

First on the list is adding to and improving the FAA’s network of weather stations to offer real-time data to pilots.

A Wright Air Service Cessna Caravan was carrying a pilot and 8 passengers on a scheduled run to Huslia Monday afternoon when it experienced trouble.  

Investigators flew over the site on Sunday, but poor weather and low visibility hampered efforts to land until Wednesday, when the weather began to clear.

The NTSB says the plane flipped on takeoff after hitting a buoy.
